Transfering Medical Records
To ensure you recieve the best medical care possible, it is crucial that all medical records you have from other medical centres are transfered to the Omineca Medical Clinic.
To transfer medical records, please come into the clinic and speak to one of our receptionists and bring with you the following information:
- Name of doctor(s) from your previous medical centre. Include the FIRST and LAST name of your doctor(s).
- Name of the town you lived (or where the medical centre is).
- Phone and fax numbers of the medical centre.
